About South Jersey Geriatric Mental Health Counseling and Care

Established in 2000, our goal at South Jersey Geriatric Care, PC is to help our elderly clients maintain their independence and support their families by reducing the stress of care giving.

Our Geriatric Care Managers will help you care for yourself, and your loved ones, by providing respectful, individual attention and resource management to give you peace of mind.

We provide licensed professional counseling services for the elderly and their family members to address:

Lori A. Heberley, LPC, CMC

SJGC President

Lori is a licensed Professional Counselor (LPC) specializing in the emotional challenges of aging. She is certified in both dementia and grief and is a member of the American Counseling Association. She can be found online at Psychology Today, and Therapist.com 
 
Lori graduated with honors from The George Washington University where she received her Master’s Degree in Community Counseling. In 2000, she created South Jersey Geriatric Care, P.C. to provide clients with respect, safety, and enlightenment as they, and their loved-ones, experience the aging process.
 
Lori provides counseling to adults to address illness, age, caregiving, and the many changes which accompany these challenges. 

Lori has 20+ years or experience as an Aging Life Care Professional, or Geriatric Care Manager. She is Care Manager Certified (CMC) as an Advanced Professional Member of the Aging Life Care Association. 
 
Lori’s experience evaluating seniors’ needs and coordinating resources for the elderly, while working with their families toward realistic and respectful goals, is the backbone of her counseling practice. Lori’s expertise in mental health counseling, with the experience provided by navigating families through the emotional and functional journey of caregiving, has given her insight and empathy to provide productive and caring counseling sessions to adults, geriatric clients, and caregivers.

What is an Aging Life Care Professional?

An Aging Life Care Professional is often called a “geriatric care manager”. Geriatric Care Management is a profession often unknown until a caregiver becomes overwhelmed and recognizes that help is needed.

A Geriatric Care Manager (GCM) is a social worker who helps seniors and their caregivers to understand an elder’s abilities and deficits in an effort to design solutions.  The goal of care management is to keep a client in the life of their choice for as long as possible. When more care is required, a care manager can assess what level of care is needed, and help the senior, as well as the family caregivers, to understand and obtain options for the required level of care.
